Hey Jac,

I think what you have seems ok, what is the ID number for? I don't think we have anything like that in Australia...?

Basically in the form you should be collecting all the information you need to;

a) Perform the transaction
b) Be able to market, and track your sales

Also, you need to be aware of privacy laws and how you look after the individuals personal data, and they have to agree to that.

It might be worth reading up about Australian privacy laws and what you are required to do when collecting personal data.

Well, you forgot phone number and fax should be optional not required. You want people to give one work/home/mobile number, not all three.

We don't have ID cards here, unlike Malaysia.

I would never reveal age on a form.

But then how do you guys identify one another? Through passport? Anything?

Because these info are vital in identifying one person to ensure they are 'real' or at least exist in australia.

For me, for example, malaysian IC would be 950213-02-****

95 - year of birth, 02 month 13 day and 02 is my state followed by 4 digits to identify i am citizen no. *****

It does sound that government are treating us as guineapigs but i guess its safer to know who's who anyway :)

Please help! No idea how to know or identify an australian!! :(

Financial institutions require "100 points" of ID. For example, a driver's license with photo is worth 70 points, a rates notice with residential address is worth 20 etc.

At the end of the day, your birth certificate would be the key orignal identity item, as most other items would be derived from this - i.e. passport, citizenship papers (if foreign), bank accounts, etc.

Gee, I hope you are not using you real name, real photo, and real date of birth on these posts. It's enough info for most indentity thieves to get to your bank accounts, credit cards, medical records.... :confused: ...or commit a crime on your behalf, so that when you say travl to Australia, you might find you are blacklisted, as someone has done something and pretended to be you....
