Looks good.. and its great that you are capturing that sort of information so early on in business!

I think with the form, as long as its telling you everything you need to know then its fine. I think if it has too much information then people wont be bothered filling it out.

Make sure that you send it to every client though, you should make it law that as soon as a job is finished a survey is sent to them the next day asking how they enjoyed your service.
